Event Description
It was reported that during set up for a cori assisted surgery, that the console screen would not turn on.They checked all relevant cable connections, attempted to start up the console with only the tablet plugged in.Also tried to connect video via display port rather than with hdmi.It seemed as though it was stuck in the boot process.The orange lights around the console were blinking and never solidified.The surgery was cancelled and rescheduled for (b)(6) 2022.Patient was not under anesthesia and was never in the room.

H3, h6: the real intelligence cori, rob10024, (b)(6), intended for use in treatment was returned for evaluation.A relationship between the reported event and the device was established.Nothing was identified visually that contributed to the reported event.A functional evaluation was performed.The reported problem was confirmed.The graphics card has failed and will not communicate to the display on any of the 4 ports.In addition, the motherboard also failed and will not allow the system to boot.A functioning graphics card and motherboard were installed and the system booted properly and displayed images.The most likely cause of this event is a graphics card and motherboard failure.A review of manufacturing records indicate the software met all specifications upon release into distribution.A complaint history review for similar reported/confirmed complaints found similar events.A historical escalation event review was completed.A review of prior escalation actions found no actions applicable to the scope of this case.The failure mode and associated risk have been anticipated within the risk file and the documented risk level is still adequate.Although no further containment or corrective action is recommended or required at this time, all complaints are monitored through complaint investigation and post market surveillance activities.
