Event Description
It was reported that a revision was completed for (4) creo mis locking caps loosening at l5 post operatively.This event occurred in (b)(6).
 
Manufacturer Narrative
Post-operative imaging was returned and a single loose locking cap was observed.Evaluation of the returned devices show wear markings on the bottom of each locking cap.This observation indicates loading likely occurred primarily around the cap outer region which would suggest that those screw heads had not normalized to the rod.This may have contributed to the reported loosening; however, an exact cause could not be determined.
