|
Information was received from a healthcare provider regarding a patient receiving fentanyl 1000mcg/ml at 983mcg/day via an implantable pump.The indication for use was spinal pain.The healthcare provider reported seeing a motor stall message after reading the pump prior to refill.The logs show the pump stalled twice on 06-mar and then again on 08-mar and has not recovered since.The healthcare provider stated the patient has not heard the alarm and the healthcare provider has been there more than 10 minutes and has not heard the alarm either.

Additional information received from the healthcare provider (hcp) stated the cause of the patient/hcp not hearing the pump alarming was not determined.It was mentioned that the patient was not yet scheduled for replacement to determine overall cause of motor and pump stall.The pump was turned off on (b)(6) 2022.Patient is now on oral and patch narcotic for pain control.The pump was scheduled for explant.

Additional information was received from a healthcare provider (hcp) via a company representative (rep).The pump was empty due to motor stall on (b)(6) 2022, and pump duration was (b)(6) 2022.There was low reservoir on (b)(6) 2022 then empty on (b)(6) 2023.They are replacing the pump on (b)(6) 2023.The pump will be sent for analysis.
