|
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was receiving via an implantable pump for non-malignant pain.It was reported that the pump connector of original catheter could not be properly detached from old pump.No known factors have led to the issue.It was reported that when squeezing the grey ovals to release the pump connected, the piece would not budge.As the physician continued to pull, it was clear the piece was starting to rip and that it was not going to come off.The white outer piece of connector broke off and catheter was still attached to pump.The only option was to cut it.A new 8784 was opened and collect was used to attach new pump piece which was then connected to new pump.The issue was resolved, patient status was alive no injury.
