It was reported by the sales rep that during an unknown surgery on (b)(6) 2022, it was observed that the expressew iii ac+ gun device was locked.During in-house engineering evaluation, it was determined that the device had a broken hook; and a slightly loose upper jaw when the jaws were closed; therefore, it showed that the jaws did not close normally contributing to the holding issue.Another like device was used to complete the procedure.There were no adverse patient consequences nor surgical delay reported.No additional information was provided.
