Product analysis: visual inspection showed that the tma was separated from the device.Additional visual inspection showed evidence of adhesive residue on the tma port.During the cleaning process there was a leak from the tma port.The reason for return was confirmed.If information is provided in the future, a supplemental report will be issued.
 
Event Description
Medtronic received information that during post-op, the customer reported that the temperature probe came out after bypass and blood poured out of the fusion oxygenator. the perfusionist was able to press it back in and hold it until disconnecting from the patient.The line pressures were 330-400 mmhg during the case. the oxygenator was used to complete the procedure.There was no reported patient impact associated with this event.Additional information confirmed that a minimal amount of blood was lost due to leak.The customer stated that no blood transfusion was required as the case was over when the event occurred.
