Event Description
It was reported that a patient experienced a blood loss event during a continuous venovenous hemodiafiltration (cvvhdf) treatment with citrate anticoagulation on a prismax control unit.Approximately 48 minutes into therapy, the touch screen was frozen/unresponsive.It was not possible for the calcium syringe to be replaced.The treatment was ended without the extracorporeal blood being returned to the patient (217 ml of blood).No patient symptom was reported at the time of the event.On the following morning, the patient received a blood transfusion.No additional information is available.

Additional information added to h3, h6 and h10.H10: the device was inspected on-site by a baxter qualified technician.A functional check after the frozen screen was successful.The event history log review showed the treatment was interrupted after approximately two days, when the operator pressed the shutdown button, likely due to impossibility to clear alarms "t0775-access extremely negative" and "t2228-anticoagulation suspended", which was caused by a touchscreen issue leading to the frozen screen.The reported condition was verified.A service history review was performed and revealed that the device has no previous service events; therefore, servicing did not cause or contribute to the reported event.Should additional relevant information become available, a supplemental report will be submitted.
