Event Description
|
This is filed to report a leak.It was reported this was a procedure to treat functional mitral regurgitation (mr) with a grade of 4+.The steerable guide catheter (sgc) was inserted.However, during advancement, the tip of the sgc became kinked and loss of fluid column occurred.Therefore, the device was removed and replaced.One clip was then successfully implanted, reducing mr to a grade of 2.There were no adverse patient effects and no clinically significant delay in the procedure.No additional information was provided.

The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to the reported event.Additionally, a review of the complaint history identified no other complaints from the lot.The investigation was unable to determine the cause for reported deformation due to compressive stress (kinked) on the sgc tip and reported leak/splash for loss of fluid column during procedure.There is no indication of a product issue with respect to manufacture, design, or labeling.
