This is filed to report the steerable guide catheter (sgc) becoming caught while advancing to the superior vena cava and dissecting the vein.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 3-4.The sgc was inserted in the patient and while advancing up to the superior vena cava, it got caught and dissected the vein.Treatment was not warranted, and the procedure continued.One mitraclip was implanted reducing the final mr to a grade of 1.There was no clinically significant delay in the procedure and no adverse patient sequelae.No additional information was provided.
