Event Description
|
Information was received from a patient who was receiving bupivacaine (dose unknown), fentanyl (dose unknown), and dilaudid (hydromorphone) (dose unknown) via an implantable pump for non-malignant pain.It was reported that the patient received a replacement personal therapy manager (ptm) - see rtg0295360.It turned out that the external device was fine.Ultimately, they discovered that it was the pump that had flipped.The patient stated that the healthcare provider (hcp) had flipped the pump back. no patient symptoms or complications were reported.

Additional information was received from a healthcare provider (hcp) who reported that they couldn¿t read the pump, so they looked at it under x-ray.They then flipped it back and it worked well.There were no external, environmental, or patient factors that caused or contributed to the event, and the event was resolved.
