In addition to the broken bending section and missing bending section cover, service found the bending tube was deformed, the universal cord was damaged, the cable tube unit was scratched, the scope connector cover was broken, and the universal cord was peeled off.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available, this report will be supplemented accordingly.Investigation activities have been opened to manage the actions related to this issue and any required mdr reporting.
 
Event Description
The customer reported the bending section was deformed and damaged.There was no report of patient or user injury associated with this event.The device was returned to an olympus service center for evaluation.Upon inspection and testing of the returned device, it was observed the bending section was broken and metal was sticking out.In addition, the bending section cover was missing.This report is being submitted for the malfunctions found during evaluation of the device (broken bending section, metal sticking out, and missing bending section cover).Additional details have been requested regarding the reported event.At this time, no additional information has been provided.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been more than 3 years since the subject device was manufactured.Based on the results of the investigation, it can be assumed the user applied excessive force to bending section, and bending tube was broken.A definitive root cause cannot be identified.The instructions for use (ifu) instruction manual state: ifu warns against this event as follows: ¿users can detect the suggested event properly by handling the device in accordance with the following ifu.Inspection of the endoscope: inspect the external surface of the entire insertion section, including the bending section and the distal end for dents, bulges, swelling, scratches, peeling of coating, holes, sagging, transformation, bends, adhesion of foreign bodies, missing parts, protruding objects, or other irregularities.Do not operate the angulation control lever with excessive force in a narrow space to the opposite direction from the bending direction while the distal end of the endoscope is not moved.The bending section may be damaged.Check the tip position of the endoscope and the shape of the bending section using fluoroscopy, etc.Do not insert the insertion tube with excessive force and twist.¿ olympus will continue to monitor the field performance of this device.
