The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Based on available information, the cause of the reported perforation could not be determined.The reported perforation as listed in the instructions for use is a known possible complication associated with mitraclip procedures.The reported minor injury/ illness / impairment was the result of case-specific circumstances.There is no indication of a product quality issue with respect to manufacture, design, or labeling.
 
Event Description
This is filed to report the septum injury.It was reported that this was a mitraclip procedure to treat degenerative mitral regurgitation (mr) with a grade of 4.The steerable guide catheter (sgc) was advanced to the mitral valve fine.When advancing the mitraclip delivery system (cds); the clip got caught under the valve.The clip was freed without causing injury, and the clip was implanted.The left atrial pressure dropped from 63 to 43 (v-wave).Mr did not change and remained at 4.The physician stated the cause of the clip getting caught under the valve was because the septum tore and a bi-directional shunt was noted.The septum tore due to poor tissue.The septum and shunt were left untreated.The patient is stable.There was no clinically significant delay in the procedure.No additional information was provided.
