Event Description
Physician used a chocolate pta balloon during treatment of a calcified lesion in the patient's mid anterior tibial artery (ata).Sight vessel tortuosity and moderate calcification are reported.A spider fx embolic protection device was used.A non-medtronic inflation device was used for balloon inflation.There was no damage noted to packaging, i.E.Shelf carton, hoop/tray.There were no issues noted when removing the device from the hoop/tray.The device was prepped as per ifu without issue.The device was passed through a previously deployed stent.No resistance was noted during advancement.It is reported the balloon is very distorted and part looks to be unattached to catheter.No issues noted with the spider filter.It appeared as if cage was separating from the balloon shaft.Detachment occurred in vessel and was removed with no issues.Detachment was not noticed until removed from sheath.This was noted post use when removed from the patient.Inflations looked normal under fluoroscopy.All components were accounted for.No patient injury reported.
