|
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was receiving compounded baclofen (500 mcg at unknown dose)) via an implantable pump for intractable spasticity.It was reported that after failed aspirating attempt, they noticed that the catheter was sutured to the pump.Once the catheter was freed from suture, aspiration was attempted again and failed.They then went to the back where they noticed that the catheter was not in the intrathecal space.The catheter had migrated out of the space.At that time ((b)(6) 2022), they revised the catheter.There were no known factors that may have led or contributed to the issue.The issue was resolved at the time of report.The patient's weight and medical history were asked but unknown.The patient's status at the time of report was alive - no injury.
