The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Based on the available information cause of the reported thrombus cannot be determined.Additionally, the reported thrombus, as listed in the mitraclip instructions for use (ifu), is a known possible complication associated with mitraclip procedures.The reported unexpected medication and hospitalization were a result of case-specific circumstances.There is no indication of a product quality issue with respect to manufacture, design, or labeling.
 
Event Description
This is being filed to report the thrombus.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 4.The steerable guide catheter (sgc) was advanced into the patient when a blood clot was noted in the right atrium (ra).Heparin was administered as protocol to achieve adequate act.With several doses of heparin given act never was greater than 250 as needed for the procedure.The sgc was removed and the decision was made to stop procedure due to the blood clot still in the ra.The patient remained hospitalized for further monitoring of the blood clot.No additional information was provided.
