The customer reported that their olympus visera pro xenon light source was experiencing the light source suddenly drop during a therapeutic procedure.According to the initial reporter, the procedure was completed with the subject device, but with a delay.The user had to manually increase the amount of light as needed.There was no harm or negative impact to the patient.

The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user's report was confirmed.The light source was suddenly going dark during testing.Additionally, a scope socket sensor was noted worn.If additional information becomes available following the device evaluation, a supplemental report will be filed.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.It has been over 11 years since the subject device was manufactured.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, a root cause could not be identified, however, misuse from handling was cited as likely cause but cannot be conclusively determined.Olympus will continue to monitor field performance for this device.
