Event Description
During an atrial fibrillation procedure, the pericardium was perforated with the needle during the transseptal puncture.The guidewire was noted to be going around the heart, therefore the introducer was not inserted to prevent a potential increase in the perforation and the procedure was stopped.The physician believes the perforation occurred due to anatomical abnormalities and was not related to the performance of the transseptal needle.The patient was in stable condition.

Additional information g3, h2, h3, h6 the results of the investigation are inconclusive since the device was not returned for analysis.Based on the information received, the cause of the reported pericardial perforation could not be conclusively determined.Review of the device history record was not possible as the lot number is unknown.
