This is filed to report the leak in the hemostasis valve.It was reported that, during preparation of the steerable guide catheter (sgc), fluid leaked from the hemostasis valve.The stopcock was replaced but the hemostasis valve still leaked.A crack was noted in the valve; therefore, it was replaced with another sgc.There was no patient involvement.There was no clinically significant delay.No additional information has been provided.

A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no other complaints reported from this lot.Based on all available information, the leak is a cascading event of the cracked flush port.The cracked hemostasis valve flush port appears to be related to a potential product quality issue.The investigation was unable to determine a definitive root cause for the issue.This issue is being addressed per internal operating procedures.Abbott will continue to trend the performance of these devices.
