|
Information was received from a healthcare provider via a company representative regarding a patient receiving morphine 10mg/ml at 2mg/day and bupivacaine 40mg/ml at 8mg/day via an implantable pump.It was reported that the patient wasn¿t receiving medication through normal use of the pump, the patient wasn¿t getting medication as dosed.The catheter was checked for patency and found to not be patent.There were no environmental, external or patient factors that may have led or contributed to the issue.The diagnostics and troubleshooting performed was a dye study.A catheter revision was performed.It was found that the catheter connection was not secure resulting in ineffective delivery of medication.The pump was reconnected to catheter and found to be working as normal.The issue was resolved at the time of the report and it was noted that the healthcare provider would not have any further information regarding the event.
