Event Description
According to the reporter, during the laparoscopic umbilical hernia, when the mesh was introduced into the patient via a trocar, the surgeon noticed a dime size tear in the mesh and it was removed.The mesh was not hydrated, nor was it cut to accommodate the defect.Another mesh was used to complete the case.There was no patient injury.
