Event Description
It was reported that during a cryo ablation procedure, the needle was used to cross septum successfully.The guidewire would not advance into the sheath.The dilator was removed and the sheath was flushed.Small fragments of plastic were seen come out of the sheath.The needle scraped the inside of the sheath when it was being removed. the case was completed with radiofrequency.No patient complications have been reported as a result of this event.
