Device evaluation summary: visual inspection shows the bottom cap is damaged/broken. The portion of the bottom cap the seals off the water side is cracked/broken. When the water side of the device was connected to a di water supply and was engaged there was di water leaking from the bottom of the device. Reason for return was confirmed. Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803. This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda. Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿. Event Description
Medtronic received information that prior to use of a fusion oxygenator, the customer reported that during priming of the system, fluid was noticed coming out of the bottom of the oxygenator. The device was replaced and a crack was observed on the bottom of the device on the inside layer of the plastic. There was no patient involvement so no adverse effect occurred. Medtronic received additional information that there were no damage to the packaging and no other devices were damaged in the same box/shipping container.
