Event Description
This is being filed to report the kink in the soft tip.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 4.While inserting the steerable guide catheter (sgc) into the femoral vein resistance was met and the soft tip kinked and broke sightly.The sgc was removed without issue and replaced with a new one.One clip was implanted, reducing mr to 1.There was no clinically significant delay in the procedure and no adverse patient effects.No additional information was provided.
 
Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to the reported event.Additionally, a review of the complaint history identified no other complaints from the lot.All available information was investigated and based on the information provided, the difficult to insert the sgc into the anatomy resulting in soft tip kink and soft tip break appears to be related to patient anatomy and due to severe vein tortuosity.There is no indication of a product issue with respect to manufacture, design or labeling.
