Event Description
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was receiving an unknown drug via an implantable pump for unknown indications for use.It was reported that the 8780 catheter was not implanted, a new catheter was used.The healthcare provider felt the catheter was not defective.No patient issues were reported.The patient's weight and medical history were asked but were not made available.The patient was reported alive with no injury.Additional information was received that there was a procedure issue during the system replacement as the hcp pulled back on the catheter while the needle was still engaged and fractured the catheter.
