H.10: through follow-up communication livanova learned that after a couple of weeks from the installation in june 2022 heater cooler 3t system started to generate alarms on their theatre monitoring system.At that point the unit failed the insulation test.A livanova field service representative was dispatched to the facility to investigate the device and could confirm the reported issue.The failure was traced back to a defective heater element on the cardioplegia circuit.The part was replaced and the problem could be solved.Subsequent functional verification testing was completed without further issues and the unit was returned to service.Complaint database review didn't identify short circuit events related to early failure of electrical safety test failure for high earth leakage current in last 3 years.Based on above, and considering that unit was manufactured in 2022, it cannot be ruled out that the reported issue was most likely due to heater element early failure.
