Event Description
The actis calcar planar spun the #2 broach and caused the femur to fracture.The femur was cabled and we switched to the corail stem and the hip was closed.There was a 20 minute surgical delay.

Product complaint # (b)(4).Investigation summary: the device associated with this reported was returned.After physical and photo review, no defect or malfunction was identified in the device.The seized condition reported cannot be determined due to the mating device was not returned.No issues were observed, therefore the allegation is not confirmed.Depuy considers the investigation closed.Should additional information be received, the information will be reviewed and the investigation will be re-opened as necessary.Device history lot : the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation mre was not performed.
