The device has not been returned/received to date.If the device is received, a supplemental report will be submitted with the investigation results.We are unable to determine if any product condition could have contributed to the reported thermal issue with the controller.Lot release records were reviewed and the product lot met all acceptance criteria.Specifically, a pod is paired to a pdm and put through simulated use testing including communicating with the pdm, deployment, delivering fluid, occlusion detection, and freedom from hazard alarms.
 
Event Description
The patient reported that when their controller was sitting on the table, they heard a vibration, a "pop" sound and there was a burning smell.
 
Manufacturer Narrative
The returned device was evaluated and an unknown residue was observed on the device.It cannot be conclusively determined if this residue was caused by a thermal event.The data and device history presented no alarms.The battery was observed to be swollen.The cause of the swelling could not be determined.It could not be determined if the swelling of the battery caused the unknown residue observed on the pdm.The screen was observed to be damaged.This did not affect the functionality of the touch screen.
