This report is being submitted in pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by depuy mitek or its employees that the report constitutes an admission that the device, depuy mitek, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.Additional narrative: udi: (b)(4).To date the device has not been returned.If the device or further details are received at a later date a supplemental medwatch will be sent.
 
Event Description
It was reported by a healthcare professional in china that during a repair of lateral collateral ligament of ankle joint procedure on (b)(6) 2022 , it was observed that the anchor on the mini qa+ w/ #0 ocord and os-2 needle w/bit device was broken off.Removed all the broken parts.Another like device was used to complete the surgery.There were no adverse consequences to the patient nor surgical delay reported.No additional information could be provided.

This report is being submitted in pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by depuy mitek or its employees that the report constitutes an admission that the device, depuy mitek, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial medwatch, a follow-up medwatch wil be filed as appropriate.H10 additional narrative: investigation summary: the device was returned to mitek for evaluation.Mitek then conducted visual inspection of device received.Upon visual inspection, it was found that the anchor came off from the tip and it was broken; also, wear marks were found; therefore, this complaint can be confirmed.The suture was slighly damaged.Manufacturing record evaluation is not required as the reported event is not associated with the manufacturing process and/or the potential cause of the defect cannot be associated to manufacturing.As part of depuy synthes mitek quality process all devices are manufactured, inspected, and released to approved specifications.The possible root cause could be related when twisting or applying bending force to the inserter as either may damage the anchor, suture or inserter tip.As per ifu, moving the power drill unit off the axis of the hole while drilling may cause the drill tip to break.Do not twist or apply bending force to the inserter as either may damage the anchor, suture, or inserter tip.Incomplete insertion or poor bone quality may result in anchor pullout.Excessive tension may overload the anchor or suture.At this point in time, no corrective action is required, and no further action is warranted.However, depuy synthes mitek will continue to track any related complaints within this device family as a means of monitoring the extent with which this complaint is observed in the field.Device history lot : manufacturing record evaluation is not required as the reported event is not associated with the manufacturing process and/or the potential cause of the defect cannot be associated to manufacturing.
