This is filed to report a thrombus and intervention.It was reported a mitraclip procedure was performed to treat degenerative mitral regurgitation (mr) grade 4+ with a flail.While advancing the steerable guide catheter (sgc), a thrombus was observed on the guide tip.The physician tried twice to advance the guide, however, the thrombus remained on the tip of the guide.Aspiration was attempted to remove the thrombus, but the entirety of the thrombus was not confirmed to be removed.The procedure was then aborted and the patient was placed on anticoagulation medicine.The mr was grade 4+.No additional information was provided.
