Event Description
It was reported when using the bd vacutainer® sst¿ ii advance plus blood collection tubes, the device experienced excessive additive quantity.This event occurred once.The following information was provided by the initial reporter.The customer stated: the cerballiance vauréal site received an order on 14/09/2022 order no.(b)(4) for the product ref: 367957, lot: 2192102, exp: 31/01/2024 with excess gel.We would like to know if this has any impact.We are concerned that the excess frost will impact our automats.

Date of event: unknown.The date received by manufacturer has been used for this.A device evaluation is anticipated, but has not yet begun.Upon completion of the investigation, a supplemental report will be filed.

H.6.Investigation summary: bd had not received samples, but one (1) photo was provided for investigation.The photo was reviewed and the indicated failure mode for excess gel was observed.Additionally, one hundred (100) retention samples from bd inventory were evaluated by visual examination and the issue of excessive gel was not observed.Based on a review of the device history record for the incident lot, all product specifications and requirements for lot release were met.There were no related quality issues during manufacturing of the product.This complaint has been confirmed for the indicated failure mode excess gel.Bd was not able to identify a root cause for the indicated failure mode.
