Event Description
|
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was receiving dilaudid (hydromorphone) (20 mg/ml at unknown dose) via an implantable pump for unknown indications for use.It was reported that on (b)(6) 2022, the pump should have had 3.2 ml and they aspirated 12 ml.Initially the patient reported no symptom changes but later the patient reported that their "back was hurting more than usual".The patient also reported a fall about a week ago along with reporting that they have had their pump flip in the pocket multiple times.The pump event logs were pulled and there were no issues with the pump.It was reviewed that if the pump was flipping in the pocket that could lead to catheter occlusion issues with it coiling.It was reviewed to consider catheter diagnostics as a next step.

Additional information was received from a healthcare provider (hcp) via the company representative who reported that no diagnostics /troubleshooting had yet been done.The cause of the volume discrepancy was unknown.The issue was not resolved, and no action was planned at this time.
