Medtronic received information that during the implant of this transcatheter bioprosthetic valve, the valve was "antecedently expand ed", therefore was recaptured.The valve was implanted."it does not seem to be post-expansion." three years post implant, an echocardiogram revealed trivial aortic regurgitation (ar).Approximately four years post implant, moderate to severe ar was reported as a result of unspecified structural valve dysfunction (svd) and a leaflet tear.The valve was explanted and replaced with a surgical aortic valve.A macroscopic, severe leaflet tear was reported by the explanting physician.The explanted valve was sent for pathological evaluation.No additional adverse patient effects were reported.

Additional information was received which reported that the decreased in adls were a result of the thoracotomy.No additional adverse patient effects were reported.
 
Event Description
Additional information was received which further confirmed that there were no issues with frame expansion at the initial implant of the valve.A post balloon aortic valvuloplasty (bav) was not performed during the index procedure, however a pre-implant bav was performed on the native valve using an 18 mm balloon.It was reported that the explant was successful, and the patient was alive, currently under-going rehabilitation at another hospital due to a slight decline in activities of daily living (adl).No additional adverse patient effects were reported.
