Event Description
It was reported that when trying to remove the left ventricular (lv) lead from the cardiac resynchronization therapy defibrillator (crt-d) header,  the wrench was unable to engage the set screw. The physician removed the grommet, and presumably the set screw as the set screw was never found. At that time, the left ventricular (lv) lead was very difficult to remove from the crt-d header. The lead did come out after pulling for a few minutes. The lead tested normally and remains in use.The crt-d was explanted and replaced. no patient complications have been reported as a result of this event.
