Event Description
According to the reporter, some devices did not emit a rem alarm when the device was inserted into the monopolar 1 slot.The device was recognized, but the rem alarm did not sound.

Event Description
According to the reporter, prior to use, some devices did not emit a rem alarm when inserted into the monopolar 1 slot.The unit would recognize the device but sometimes rem alarm would not sound when it should.When the pencil was inserted into another forcetriad and another return electrodes, it was properly recognized and the rem alarm was displayed.In addition, even if the forcetriad with the problem was used and the pencil was changed to a new one and the same return electrode was used, it was recognized and the rem alarm sounded.Based on these verifications, it was determined that the unit was malfunctioning.Another forcetriad in the hospital was used for the operation.There was no patient involvement.
