Event Description
It was reported that the lead was showing high impedance and as a result the hcp cut part of the distal end of the lead and it was capped.There was no patient impact reported.Additional information was received: it was reported that the distal end was capped and the proximal end was still connected to the extension.Additional information was received: it was reported that the patient's leads were damaged/broken and there was a revision to remove them.The ins and extension were still implanted and new electrodes were placed.
