Medtronic received information that an unknown period of time following the implant of this transcatheter bioprosthetic valve, the patient died.The cause of death was not received.No further information was provided.

Additional information was received which reported that the cause of death was a left ventricle rupture.The cause of the lv rupture was not reported, however, according to the physician, neither the valve nor the delivery catheter system (dcs) contributed to the death.

Additional information was received which reported that the cause of the left ventricle rupture was a non-medtronic (safari) guidewire.The rupture was not noted until the completion of the valve implant.A sternotomy with direct repair was attempted but was not successful.
