Event Description
It was reported that the patient passed away on (b)(6) 2023 due to a stroke.The stroke was an acute to subacute large left middle cerebral artery (mca) territorial infarction with subfalcine.The patient experienced a new fixed and dilated pupil.A computerized tomography scan revealed complete occlusion of the left internal carotid artery (ica) from the carotid bifurcation to its bifurcation into the left anterior cerebral artery (aca) and mca arteries and mass effect with an 8 millimeter shift.There was no change to patient condition or anticoagulation status that contributed to the event.Related mfr report # for the lvas: 2916596-2023-00783.

Manufacturer's investigation conclusion: a direct correlation between the centrimag blood pump and the reported stroke and patient outcome could not be conclusively established through this evaluation.The lot number or other identifying information of the product was not reported and was not able to be determined during the investigation.The centrimag blood pump instructions for use (ifu) lists neurologic dysfunction and death as adverse events that may be associated with the centrimag circulatory support system.This ifu also provides the following warnings and cautions: ifu warning #7: it is intended that systemic anticoagulation be utilized while the device is in use.Anticoagulation levels should be determined by the physician based on risks and benefits to the patient.Ifu warning #10: frequent patient and device monitoring is recommended.Ifu caution #2: this device should only be used by persons thoroughly trained in extracorporeal circulation procedures.No further information was provided.The manufacturer is closing the file on this event.
