Event Description
It was reported that the patient had a return of pain in (b)(6) 2022.It was reported on (b)(6) 2023 the physician went to do the pump fill and he was expecting to take out 4.6 ml but there was 20 ml left. it was reported they did a catheter dye study but the patient does not know what the outcome of that was.It was reported due to the volume discrepancy, the physician decided not to refill the pump.It was reported the pump started to audibly alarm a single tone alarm and the patient reported the pump should be empty.The patient was redirected to their hcp to further address the issue.
 
Event Description
Additional information was received from a healthcare professional (hcp) via a company representative (rep).It was re-reported that there had been a volume discrepancy in january and that the physician didn't refill the pump at that time.The pump logs showed both a critical and non-critical alarm in january due to the physician not refilling the pump.The patient reported they had increased pain.Initially it had been thought it had been a pump issue but logs had shown no motor stalls and the catheter was unable to aspirate.The patient had been scheduled for a replacement surgery, but upon inspection of the catheter there had been some pus accumulated, so they decided to explant the entire system and re-implant a new one at a later date.Patient weight and medical history were asked but remain unknown.At the time of this report the issue had been resolved and the patients status was alive - no injury.

Event Description
Additional information received from the healthcare provider (hcp) via a manufacturer representative (rep) indicated that the cause of the previously reported pus was unknown.The pus sample came back negative for an infection.In regards to actions and interventions taken to resolve the observed pus, the entire system was explanted and the patient was observed in the hospital.It was also reported that an end of service (eos) alarm occurred.
