Event Description
It was reported that during priming with a revaclear, a foreign particle was observed inside the dialyzer.There was no patient involvement.No additional information is available.

H10: the device was received for evaluation.The dialyzer was observed primed with the solution.During visual inspection, a black substance was observed inside the housing.After the dialyzer was opened to further investigate the particulate matter, the fibers were noted to be burned and not particulate matter.The reported condition was verified.The cause of the condition could not be determined; however, the most probable cause was due to hot blades and thermoforming during manufacturing.If there is improper or incomplete thermoforming of the fiber bundle, this can result in a bad cut at hot blade, leading to burned fibers.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
