|
Information was received from multiple sources (company representative, consumer, healthcare professional) regarding a patient receiving dilaudid 10 mg/ml and fentanyl 5000 mcg/ml (doses unknown) for non-malignant pain via an implantable pump.It was reported the patient had heard a non-critical alarm at 5:30 pm on (b)(6) 2023 for every hour until 6 am the following day.It was reported the patient was at the physician's office and they were unable to get telemetry to read the pump.It had been confirmed the patient did not have a recent mri or emi exposure.The patient had no reported symptoms.The company representative stated they tried using 2 different clinician programmers, changing the batteries on the communicator, leaving the cord between the programmer and communicator plugged in, repositioning the patient, and moving rooms, but the issue persisted.Technical services had them check previous reports to confirm early replacement indicator is 2023-apr.Technical services confirmed the patient did not have a personal therapy manager (ptm).They conference the healthcare provider at the office with the patient.The healthcare provider stated the patient heard their pump alarm again and it had been a critical alarm tone.The patient then clarified the critical alarm was the same one they had heard previously.Technical services reviewed troubleshooting for telemetry issues and attempting to pair with a ptm to see if telemetry was available that away.The company representative had then disconnected and the issue was unresolved.

Additional information was received from a healthcare provider (hcp) via the company representative who reported that the cause of the alarm and the cause for being unable to get telemetry with the pump was not determined.Additional troubleshooting included placing the communicator at 6 o¿clock on the pump and raising the communicator a centimeter above the pump.Attempts were made again today to get a reading, but the issue was not resolved.The plan was to explant the pump.

Event Description
|
Additional information received from the healthcare provider (hcp) via a manufacturer representative (rep) indicated that the patient was experiencing a pump alarm on (b)(6) 2023.The hcp was unable to interrogate the device, despite trying multiple programmers and different rooms.The patient experienced withdrawal but, at the time of this report, had recovered.On (b)(6) 2023, the hcp was able to interrogate the pump and discovered that a pump memory error had occurred and reset the pump to minimum rate.No additional actions were taken.A pump explant was planned and "not replaced at a date to be determined later".It was indicated that the date of the planned explant was (b)(6) 2023.It was noted that the pump would be returned to the manufacturer.It was unknown if any environmental, external, or patient factors that may have led or contributed to the issue.At the time of this report, the issue was resolved and the patient status was "alive-no injury".It was noted that the patient hada medical history of chronic pain.The pump was used to deliver dilaudid (hydromorphone) 10mcg/ml at 0.063mg/day and fentanyl 5000mcg/ml at 31.5mcg/day.
