Event Description
According to the reporter, intra-operatively of a laparoscopic procedure, when trying to fire the device to anastomose the mesentery.The magazine has loosened and broke off into the patient.The magazine was replaced but the same issue occurred on the second magazine.The disengaged magazines were recovered with normal access.There was no patient injury.

Event Description
According to the reporter, intra-operatively of a laparoscopic procedure, during fobi ring removal, there was the incidental finding of the mesentericum.When trying to fire the device to anastomose the mesentery, the magazine loosened and broke off into the patient.The magazine was replaced but the same issue occurred on the second magazine.The disengaged magazines were recovered with normal access.There was no patient injury.
