Event Description
According to the reporter, during laparoscopic esophagus resection, when closing ductus and artery of gallbladder, there were clips were formed properly while some inner clips were formed like an x and did not fix on the tissue.Incompletely closed clips were removed from the tissue.There was no patient injury.

Event Description
According to the reporter, during laparoscopic esophagus resection, when closing ductus and artery of gallbladder, there were clips were formed properly but did not release from the black house - clips were closed but did not move out.Meanwhile, some inner clipswere formed like an x and did not fix on the tissue.Incompletely closed clips were removed from the tissue/picked out the abdominal wall.There was no patient injury.
