This device has been returned for evaluation.After checking the appearance, no abnormalities were found in places that could be visually confirmed.The device was turned on for about 10 minutes, but the power failure did not occur.No abnormality was found in air supply.It was confirmed that the cautio lights up and the caution sound sounds when the cover is closed with a hand.It was also confirmed that the caution light turns off and the caution sound stops when the hand is released.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or when additional information becomes available.
 
Event Description
The customer reported to olympus that when pneumoperitoneum was started with this high flow insufflation unit during laparoscopic appendectomy, the power suddenly turned off after ten (10) minutes.The user turned it on again and used it for about five (5) minutes, but the power went off again.The user tried to turn it back on, however, it would not.The user switched to the facility-owned high flow insufflation unit and completed the procedure.The procedure was delayed for 3-5 minutes due to the turning the power on and replacing the device.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and to correction the initial with information inadvertently left out.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely the phenomenon occurred due to the failure of the main substrates.However, the root cause of the failure could not be identified.Olympus will continue to monitor field performance for this device.
