The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user¿s report was confirmed.One of the two tubes located inside the unit had detached and caused the reported error.There was no notable damaged to the tubing or surrounding area.The tube was reinserted, test-pulled, and could not easily be removed.Following the reattachment of the tubing, the alarm was no longer present, and the unit was functioning as expected.If additional information becomes available following the device evaluation, a supplemental report will be filed.
 
Event Description
The customer reported that during preparation for an abdominal surgery, it was noticed that his olympus high flow insufflation unit was displaying an error.The olympus field service engineer (fse) who was on-site at the time, noted that the device was not functioning properly and needed to be replaced in order to continue the procedure.Reportedly, the patient was under anesthesia at the time, but there was no delays or patient harm reported from this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it s likely that b tube(the tube that connects the pressure sensor of the manifold u and the main board(cr board)came off and the pressure could not be measured and it caused the front panel display to disappear and trigger an alarm.No specific factor could determine the cause of the phenomenon.Olympus will continue to monitor field performance for this device.
