Event Description
|
Information was received from multiple sources (patient, manufacturer representative, healthcare provider) regarding a patient who was receiving dilaudid (hydromorphone) (15mg/ml at 5.1mg) and bupivacaine (15mg/ml at 5.1mg) via an implantable pump indicated for spinal pain.It was reported that on (b)(6) 2023 the patient had their entire system replaced.The rep reported that the patient had two volume discrepancies at 2 of previous 3 refills in clinic.Also, a csf side port study was done and unsuccessfully tried to aspirate csf.It was reported there was no csf.The dates of the refills were unknown.1 of the refills patient did have normal residual.The patient didn¿t have any withdrawal or overdose symptoms, but simply gradual loss of efficacy or therapeutic benefit.The date of symptom onset was unknown.There were no environmental/external/patient factors that may have led or contributed to the issue.The issue was resolved at time of report.The patient's weight was provided.The patient's medical history was asked, but unknown.The patient's status at time of report was alive - no injury.Additional information received from the patient reported that at time of refill there was still the full amount of medication in the pump.No further information was provided.

Additional information was received from a healthcare provider (hcp) who reported that the cause of the volume discrepancy, inability to aspirate, and no csf (cerebrospinal fluid) was suspected to be that the catheter was clogged or nonfunctional.
