Event Description
|
According to the reporter, during a laparoscopic gastrectomy, while the trocar was being used in the abdomen with a surgical stapler, the nurse noticed that the trocar interior shredded on insertion of 12mm powered stapler from another manufacturer with seamguard reinforcement system.The balloon tip trocar was replaced with a trocar from another manufacturer and the procedure continued.There was no patient injury.

According to the reporter, during a laparoscopic gastrectomy, while the trocar was being used in the abdomen with a surgical stapler, the nurse noticed that the trocar interior shredded on insertion of 12mm powered stapler from another manufacturer with seamguard reinforcement system.Visibly, it looks like the seal of the trocar came out through the proximal head of the trocar upon removal of the stapler with sureform.The balloon tip trocar was replaced with a trocar from another manufacturer and the procedure continued.There was no patient injury.
