Event Description
The customer reported to olympus that during preparation for laparoscopic procedure, this high flow insufflation unit turned on for 0.5 seconds and then turned off with a beeping sound, making it unusable.The patient was taking warfarin and had chosen a laparoscopic procedure, which could cause as little bleeding as possible.However, since the device could not be used, the procedure was changed to an open surgery.The device, as reported, had not been used for over a year.There were no reports of further patient or user harm associated with this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and device evaluation.The device was returned to olympus for inspection, and the customer¿s reported complaint was confirmed.In addition, per the log analysis, e03 was confirmed, and the pressure sensor was broken.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, it is likely that the reported event occurred due to a faulty pressure sensor on the main board.However, the root cause could not be determined.This supplemental report includes a correction to b5 and g2 to provide information that was inadvertently not included on the initial medwatch.An update has been made to d9 and h3 from the initial medwatch.Also, new information has been added to d8.Olympus will continue to monitor field performance for this device.
 
Event Description
The following additional information was received: it was confirmed that the laparotomy procedure was completed successfully, and there was no patient harm.
