The device referenced in this report was not returned to olympus for evaluation.The root cause cannot be determined at this time.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
A user facility reported to olympus that the high flow insufflation unit powered off 2 seconds after it was turned on during a inguinal hernia laparoscopic surgery.The surgeon tried several times but the device would not work.The surgical procedure had to be converted to an open approach.It was stated that there was no health damage beyond the surgical conversion and the surgical procedure was successful.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device history record was not reviewed since the subject device was manufactured over 15 years ago.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.Based on the results of the investigation, since the device was not returned to olympus for an evaluation, it could not be determined why the device malfunction occurred.Therefore, the root cause of the reported even could not be determined.This supplemental report includes a correction to g2 to provide information that was inadvertently not included on the initial medwatch.Olympus will continue to monitor field performance for this device.
