Event Description
It was reported that during the implant procedure of the left ventricular (lv) lead, two guidewires were attempted to be used to place the lead and a third guidewire was utilized.The third guidewire would not advance beyond the end of the lv lead and became stuck inside the lead.The physician was not able to advance or remove the guidewire.The lv lead was attempted and not used.No patient complications have been reported as a result of this event.
