Event Description
|
Information was received from multiple sources (a healthcare provider (hcp), and another hcp via a manufacturer representative (rep)) regarding a patient receiving lioresal (2000mcg/ml concentration and 1578mcg/day dose) via an implantable pump.The indication for use was intractable spasticity.It was reported that a nurse (caller) called in to the manufacturer's technical services and stated that they attempted to refill the pump today and were unable to access the refill port.Caller was concerned that the pump was flipped.Caller stated that they took a x-ray but the images were not real good.The technical services specialist (tss) discussed ap view and pump catheter connector.Tss discussed lateral view and pump electronics and bellows.Four days later, the rep reported that the patient's pump flipped in the pocket.There were no applicable environmental, external, or patient factors that may have led or contributed to the issue.The pocket was revised and moved from the left lower abdomen to the mid/upper abdomen to prevent flipping.No patient symptoms were reported.It was noted that the hcp had no further information.The patient's weight was asked but unknown.The patient's status was alive with no injury.The issue was resolved at the time of the report.
